New Patients

Welcome to The Lane Center

In an effort to better serve you, please follow the reminder checklist below to prepare for your first visit:

  • Fill out and sign all downloadable forms before your appointment. Please fax all forms to 781-741-8810 and bring the originals with you to your appointment.
  • Please allow approximately an hour and a half to two hours for your first visit and one hour for subsequent visits.
  • Please wear comfortable loose fitting clothing.
  • If possible, arrive 5-10 minutes early. This will help you to rest a few minutes enabling your pulses to normalize before your treatment.
  • Payments, in the form of cash, check, Visa or MasterCard are expected at the time services are rendered.
  • If you have insurance and wish to send them a statement for possible reimbursement, please inform our office staff so an appropriate statement can be prepared for you
  • Your appointment time is reserved especially for you. In the event that you need to change your appointment, please provide us at least 24 hours notice.
  • Do not hesitate to discuss any questions you might have with us.

Existing Patients

If you are an existing TLC patient, please choose and complete the appropriate forms to bring with you to your next appointment.

Please take time to complete these forms as thoroughly and thoughtfully as possible. The information you provide will help us to understand your health concerns so that we can determine a treatment plan that works for you.
